Responsibilities

Conducts Annual Health Assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam

 

Perform evidence-based practice screenings including point of care testing (as appropriate)

 

Identify diagnoses to be used in care management and active medical management in the furtherance of treatment

 

Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of the assessment

 

Communicate findings in the assessment that will be used to inform patients primary care provider of potential gaps in care

 

Identify urgent and emergent situations and intervene appropriately

 

Educate patients on topics such as disease process, medication, and compliance

Qualifications

Certified: California Family Nurse Practitioner License

DEA Certificate

BLS or ACLS certification

Graduate of an accredited Family Nurse Practitioner Program

National certification in his/her primary care specialty

Qualified to practice by certification in the State of California

 

Experienced FNP (particularly those who have worked in Health Centers) and those FNP who have completed family practice training are preferred

 

The base pay range for this exempt position starts at $137,000.00 a year.  Our competitive salaries are dependent on knowledge, skills and experience.  The base pay does not include other forms of compensation and/or incentives.
